Expense Request Form

Instructions:

  1. If not already loaded on your computer, you will need to download the Adobe Acrobat Reader to fill and send the form.
  2. Click on the image below to automatically download the file into your computer.
  3. Complete the form, and click the “Send” button to submit the form. (Acrobat Reader may ask for permission to open your email program.)
  4. An email message, using your email program, will be generated with the form attached.  Don’t forget to also attach your receipts!
  5. If for some reason the TO address is not automatically filled in, mail the form to “ExpenseReport@kofc7566.us”.
  6. Send the message as you normally would for any email message. The request form will be automatically submitted for you.
